Torchlight’s Collaborative Continuing Care program provides structured, ongoing recovery support and addiction case management for both clients and families—helping bridge the gap between treatment and long-term recovery. Our approach focuses on alignment, accountability, and coordination, ensuring that progress made in treatment continues forward in a meaningful and sustainable way.

A Structured Continuing Care Program

Our continuing care program is designed to provide consistent recovery support after treatment. We work collaboratively with the client, family, and treatment providers to establish a clear plan that supports long-term recovery and stability.

This includes assessing needs, coordinating resources, and maintaining consistent communication across everyone involved in the recovery process.

Before discharge, we coordinate with the treatment team to understand recommendations and prepare for next steps. From there, we develop a structured plan that reflects the client’s goals, clinical needs, and available support systems.

Personalized Recovery Planning & Resource Coordination

Each client receives an individualized recovery plan designed to support stability and progress outside of treatment. As part of our addiction recovery management and case management services, this may include:

  • Coordination with treatment providers and clinical professionals

The assigned Care Manager maintains detailed session notes. A Case Manager reviews these notes as an added layer of oversight to identify progress, challenges, and any concerns that may require timely attention.

Supporting the Family System in Recovery

Substance use disorder and mental health challenges affect the entire family system. Families often find themselves navigating uncertainty, stress, and shifting roles during and after treatment.

We work directly with the family and broader support system—including trusted friends and key individuals—to help create an environment that supports ongoing recovery. This includes guidance around communication, expectations, and accountability across the system as a whole.
