Through our intervention services and Collaborative Continuing Care model, we provide:

- Weekly structured communication and check-ins
- Accountability for both the individual and the family
- A clear framework for decision-making and next steps
Coordination Across Care
As part of Collaborative Continuing Care, we provide ongoing coordination with treatment providers and clinical teams, ensuring alignment, communication, and continuity throughout the process.
